We are excited to introduce a new Dementia Friendly Community initiative in Indiana County. You are invited to join us in creating an inclusive and supportive environment for people living with dementia, their care partners, and those important to them. Our mission is to build a Cross-Sector Community Action Team that represents the heart of our community. By engaging individuals across diverse sectors, we aim to foster collaboration and shared responsibility for meaningful change. Together, we will engage with community members to identify strengths, gaps, and priorities, creating a shared vision to guide our efforts. With an actionable plan in place, we will focus on realistic goals, track our progress, and celebrate successes while continuously improving and adapting to new opportunities. Vision: To produce an inclusive community where individuals living with dementia can be engaged and live with dignity.

Goals and Objectives
Goal #1: Establish a Community Action Team.
Objective 1a: Identify and recruit 5-6 individuals from different business sectors of the county. Members will develop an action plan for this initiative.
Objective 2b: The team shall meet on a regular basis to develop, discuss, and execute the action plan.

Goal #2: Educate our community and increase awareness.
Objective 2a: Schedule at least 6 Dementia Friends Information Sessions every year.
Objective 2b: 3 ASI staff will become trained leaders for the Dementia Live program. Offer at least 4 sessions per year.
Objective 2c: ASI staff complete memory screening training; offer the screening at public events.

Goal #3: Encourage businesses and organizations to be dementia friendly.
Objective 3a: Publicize Dementia Friendly Indiana County throughout the year.
Objective 3b: Meet with a minimum of 3 businesses per year to expand the initiative.

Goal #4: Support people living with dementia and their families.
Objective 4a: Provide information on available services and resources.
Objective 4b: Maintain a list of Dementia Friendly Indiana County businesses and organizations to distribute to individuals and caregivers.
Objective 4c: Hold monthly caregiver support group meetings.
