We are excited to introduce a new Dementia Friendly Community initiative in Dauphin, Lebanon, & Cumberland counties. You are invited to join us in creating an inclusive and supportive environment for people living with dementia, their care partners, and those important to them. Our mission is to build a Cross-Sector Community Action Team that represents the heart of our community. By engaging individuals across diverse sectors, we aim to foster collaboration and shared responsibility for meaningful change. Together, we will engage with community members to identify strengths, gaps, and priorities, creating a shared vision to guide our efforts. With an actionable plan in place, we will focus on realistic goals, track our progress, and celebrate successes while continuously improving and adapting to new opportunities. Aim/Objective: To create a community where all persons living with dementia and their care partners feel support from within their community.
We will do this through education within the community to identify and understand dementia, provide resources to better support people living with dementia and their care partners to navigate the healthcare system and planning, and engage community to create an environment that is dementia friendly.
